INTERMENT

Interment Policies and Guidelines

Client must notify the company for any interment at least seventy-two (72) hours prior to the scheduled interment. Likewise, the necessary interment fee and required payments for the lot/s shall be paid. The company reserves the right to reschedule, postpone, delay or deny interment depending on capacity to inter or availability of personnel and equipment.

Required documents / permits shall be submitted at least forty-eight (48) hours before any interment services are rendered.

Burial Permit/Tax required by law shall be on the account and responsibility of the client. In no case shall interment be allowed without the necessary burial permit. (No Burial Permit – No Interment)

Lawn lots and Mausoleum lots must be fully paid before interment will be scheduled.

Change of interment schedule and cancellation of interment shall be subjected to applicable fees.

Interment services shall be rendered solely and exclusively by Forest Lake and/or its authorized contractor.

Interment fee shall include the cost of concrete vault, grave digging, flat marble marker with engraving and basic interment setup.

Interment fee shall vary in amount depending on the nature of interment services to be rendered.
Additional service and logistics on top of the basic interment setup shall be charged to the client.

All interment shall be permanent, however transfer of remains/vaults may be allowed when deemed necessary and considered valid by the company.

There will be no interment services on January 1, Maundy Thursday, Good Friday, Black Saturday, October 31, November 1, November 2, December 24-26 and December 31.

Interment Requirements

Please have the following documents with you when you go to the park office for scheduling of interment.
  • Certificate of Rights. Lot must be fully paid. Please settle your account by clicking on this link.
  • 2 Valid ID’s of authorized transactee/ heirs.
  • Proof of Relationship (Birth Certificate/ Marriage Contract of transactee) *if applicable
  • Senior ID of deceased
  • PWD ID (if applicable)
    Certified true copy of the Death Certificate
  • Burial Permit issued by the city or municipality

IN THE ABSENCE OF THE LOT OWNER

  • Authorization letter of Lot Owner for his/her representatives to sign the interment documents
  • 2 Valid ID’s from the Lot Owner (Photocopy/ Scanned Copy with 3 specimen signatures)
  • 2 Valid ID’s of authorized representative (Actual signing the office)
  • Undertaking/ Extrajudicial Settlement (If applicable)

BONES

  • Exhumation Permit per set of bones
  • Certificate of Cadaver/ Exhumation
  • Exhumation Receipts
  • Burial Permit per bone

ASH

  • Cremation Certificate
  • Burial Permit
